Medical Questionnaire

Download and fill out this mandatory medical questionnaire, which will assess if you meet the minimum physical requirements to safely participate in this sport. The Medical Questionnaire must be completely filled and signed by a physician prior to arrival at the resort.

Check In & Sign Up
Ready for the Resort Course
After you've checked in at Beaches, stop by the dive shop for a quick orientation of our dive facility and to sign up for your resort course! The course takes an average of 3 hours, and you’ll take a swim/wade test, watch a video about diving, and practice important skills in our dedicated dive pools.

Now You're Ready
Jump in and Explore the Ocean
Now that you’ve completed your training, you're ready to dive right in and explore beneath the surface. Our professionally trained instructors will continue to keep a close eye on you, with a maximum of 4 students to every instructor in the water. You’ll find top-of-the-line SCUBA equipment included with your dive—filled aluminum dive tanks, silicon mask, and weight belts—along with a skilled team of dive professionals ready to assist in your new adventure.
